Attention patients and patient representatives: In an abundance of caution and in the best interest of our patients and employees, the walk-up windows for requesting copies of medical records will be closed at all locations until further notice. Please make your request by mail, fax, email, or electronically per the instructions below for each facility.

If you are seeking a copy of your covid-19 vaccine record, please choose "Emory Clinic" as the facility in the online tool. If you are an Emory Healthcare employee who received your vaccine through Emory Occupational Health, please use the HOME portal to print a copy of your vaccine record under the “My Medical Information” tab.

Your health record includes your medical history such as allergies, medicines and test results, as well as, health insurance information and how to contact you. It is the duty of Emory Healthcare to ensure that your record remains confidential and is maintained and released in accordance with applicable laws. To request access to your patient health information, please select an option below.

Electronic Request for Records

Emory Healthcare now offers an Online Patient Record Request tool for the following individuals to request patient records:

  • Patients requesting their own records
  • Parents of minor patients requesting records
  • Caregivers acting on behalf of a patient (i.e. Power of Attorney)

The Online tool verifies your identity by asking for a photo of your driver's license, which can be taken via webcam or smartphone. There is no additional charge to use this service.

FAQs for Request for Records

After my request for records is received, what is the timeframe for completing my request?

It generally takes 7-10 business days from the receipt of the request to complete a request for records. However, it may take a little longer if the record has not been completed by the clinician.

Is there a fee for record requests?

Yes, the fees are assessed per page and are governed by the federal government, unless the state fees are lower. Please refer to our Fee Sheet attached to the Authorization form.

If records are received by Emory Healthcare from an outside facility, are those records re-released?

For The Emory Clinic, outside records are re-released for all record request types. For the Emory Hospitals, records are re-released for legal requests only.

**Emory Johns Creek only: Outside records are re-released if the records were used to assist in treatment of the patient.

What are the hours of operations for HIM/Medical Records?

Our hours are 8:30 a.m. – 5 p.m., Monday through Friday, and closed on weekends. However, any request of records for immediate patient care from other healthcare facilities are received and processed 24/7.

**Emory Decatur Hospital, Emory Long Term Acute Care, and Emory Hillandale Hospital: 8:30 a.m. to 6 p.m., Monday through Friday. 

If I was seen at multiple Emory facilities and want my records from each of them, do I have to submit separate requests to each facility?

No, once your request is received at one facility, as a courtesy we will forward it to the other Emory facility(s) in which you are requesting records. However, it will be the requestors’ responsibility to follow up with the other facility(s), since each facility process their own records requests.

When I complete the authorization/request for records form, how long is it valid or when does it expire?

The authorization form expires 90 days from the date it is signed, unless the requestor indicates a specific date on the form.

If I want to review my records without requesting copies, can I do so?

Yes, you can review your records even if you do not wish to get copies at that time. You would contact the Release of Information section of the HIM/Medical Records department to schedule an on site appointment. Please keep in mind the Release or HIM/Medical Records staff would not be able to answer clinical questions. Should you have clinical questions, please make us aware at the time you schedule the appointment so that we can make an attempt to get you in touch with the appropriate clinical staff.

If I want my records sent to another healthcare entity/physician for an upcoming Drs. appointment, is there a fee?

No, there is no fee to the patient or the other entity when records are being sent for continued patient care. However, there would be associated fees for records being requested for personal use, attorneys, insurance companies, workman’s comp, or disability.
